Referral process

 

Do Well OT is currently accepting self referrals for private or self-managed clients or directly from HCP or NDIS support coordinators.


  1. Please complete the referral form below. If an email address is provided, we will forward you a copy for your records.  
  2. The client will be contacted within 3 business days to schedule an appointment.
  3. After the appointment is scheduled, Do Well OT will send an appointment confirmation to both the client and referrer.

What to expect

  • An initial off-road, in-home assessment is conducted to gather medical and driving history and complete assessments of cognition, vision and motor function.


  • Following the off-road assessment, an on-road assessment is scheduled and conducted with a driving instructor in their dual control vehicle.  The OT will sit in the rear passenger seat, allowing them to observe driving behaviours and provide constructive feedback.  Clients are reminded to bring their current licence, corrective lenses (where required) and well-fitting footwear to the on-road assessment.


  • Immediately following your on-road assessment, feedback will be given about recommendations that will be provided to the GP for licensing outcomes (NB the final outcomes sit with the Driver Licensing Authority)


  • A report is completed and sent to your GP who will provide a recommendation to the Driver Licensing Authority.  They will then provide you notice of the final outcome for licensing decisions.


  • For assessments which do not require a decision to be made about licensing, the process will be tailored to the individual circumstances however will likely include an initial, off-road component to gather important background information before completing on-road assessments allowing for trial and recommendations for vehicle modifications or other therapeutic interventions where required.
