Do Well OT is currently accepting self referrals for private or self-managed clients or directly from HCP or NDIS support coordinators.

1. Please complete the referral form below. If an email address is provided, we will forward you a copy for your records.  
2. The client will be contacted within 3 business days to schedule an appointment.
3. 3. After the appointment is scheduled, Do Well OT will send an appointment confirmation to both the client and referrer.

• All assessments are completed in the clients home (or at a relevant location for the concern eg local community for powered mobility aid assessment)

• Where high cost or high risk equipment has been recommended a trial is required. Trials can be arranged in-store or in the clients home, depending on client preference and equipment transportability.

• A complimentary follow up phone call is provided following implementation of recommendations - where required a face to face review can be quoted.

• Home safety assessments are generally completed in a single visit taking 1.5hrs to complete, depending on the complexity and number of concerns.

• If additional time is required a review visit may be recommended.

• Alternatively where the client would benefit from 2x shorter assessments due to fatigue, pain or cognitive load, this can be arranged.

• An initial assessment for a single item is scheduled for 1hr.

• An equipment trial is recommended for all high cost/ high risk equipment. A trial may also be recommended for other equipment depending on specific client needs eg pain, cognition, mobility to ensure a safe and effective prescription.

• A report of the assessment findings and recommendations will be provided to the referrer. Do Well OT also offer to provide quotes for the recommended equipment and home modifications (diagrams included with quote), where requested.
